How the Survey is Conducted

The survey process is customized to meet your community's goals and needs. Typically, the Minnesota Institute of Public Health provides the following services:

  • Assists you in designing the survey process to meet your needs, including deciding sample size and methodology.
  • Works with you to select a scientific, random sample of adults in your community.
  • Provides a survey cover letter to include in the survey packet and also a follow-up reminder card.
  • Analyzes the returned suveys.
  • Prepares a comprehensive report with graphics and text that clearly explains the findings and recommends action steps.

MIPH staff will work with a community contact on the logistics of the survey, which involve:
  • Review of cover letters for pre-notification letter and survey
  • Obtaining signatures for the above letters
  • Obtaining zip codes from which the sample is drawn
  • Coordinating the inclusion of an incentive to be sent with survey 

MIPH staff will coordinate:
  • Printing of all of the above (except incentive)
  • Mailing pre-notification letter, surveys, and reminder postcard
  • Entering, cleaning, and analyzing data
  • Writing final report 

Community will provide:
  • Letterhead and envelopes for pre-notification letter
  • Incentive 

The survey is a three piece mailing with each piece being sent at one week intervals:
Week 1 - Pre-notification letter to alert community member to the coming survey
Week 2 - Survey, including cover letter and self-addressed, stamped envelope, and incentive (if desired)
Week 3 - Postcard to remind community member to complete survey
